Be a part of competent and caring team of professionals providing individualized clinical stabilization and recovery services as an After Care Coordinator at the Dorchester Family Center at Behavioral Health Network!

Your experience, knowledge and great skills will support the Northern Hope Center with the following:

  • Working with treatment team and individuals to ensure successful referral to and engagement in next level of care.
  • Providing information, referrals, and follow-ups as specified in individuals' treatment plans.
  • Facilitating treatment groups for person's served.
  • Maintain consistent communication through documentation and verbal contact.
  • Complete client documentation in a timely and thorough manner.
  • Complete EIM/ESM data input.
  • Recognize and reports program specific concerns.
  • Attends and participates in staff meetings, in-service trainings, and mandatory trainings.


Who are we?

The Dorchester Family Center provides long-term residential family substance use treatment for women and their children of all ages, from newborns to teens. The diverse experienced team at the Dorchester Family Center provides a variety of services including individual and group counseling, therapy and psycho-educational groups, individualized treatments plans, and medical and mental health referrals, among others. Our goal is to provide a safe and sober living environment to allow successful family reunification and long-term family stabilization.

What are we looking for in an After Care Coordinator?
  • High School Diploma or GED equivalent (required); Bachelor's degree in related field (preferred).
  • At least one year of experience working in substance abuse treatment field (required).
  • Working knowledge of DPH Bureau of Substance Abuse regulations and referral sources.
  • Familiarity with internet, word processing and email systems.
