Education: High school diploma or GED. Bachelor's Degree in a related field is preferred.
Experience: A minimum of 2 years of experience working with families in a related field.
Supervises: None.
Summary: The Family Support Coordinator is responsible for using sound professional judgment and best practices to ensure quality services are provided to the service population while maintaining compliance with established program standards.

Essential Functions:
1. Outreach and recruitment of participants for classes.
2. Establishes and maintains a trusting and professional working relationship with families by providing regular contact through home visitation.
3. Administer intake, assessments, and screening tools to clients and family members.
4. Assessing family needs and referring to community resources as needed.
5. Scheduling and planning family activities.
6. With approval of their Team Lead and/or Director purchases basic needs for families as required.
7. Serve as liaison for family to community resources.
8. Provide transportation to appointments, required program classes/activities.
9. Meets with Team Lead on bi-weekly basis to evaluate status of families on caseload.
10. Provide parent & health education, case management, resource and referrals, basic needs support, transportation assistance, and/or other support services to families on caseload.
Measurable Deliverables:
1. Obtains and maintains certification in Safe care curriculum. Will carry a Safe Care caseload of 1-2 clients to be seen on a weekly basis.
2. Maintain current written records of all home visits, interaction, and contact with families/clients and file in appropriate clients files on a weekly basis.
3. Attend all required trainings and meetings.
4. Provide community education on child abuse prevention strategies on a monthly basis.
5. Develop together with Team Lead updates, reminders, and community education information on a quarterly basis.
Credentials, Skills and Abilities:
1. Maintain current licenses and certificates required for the position including but not limited to:
• First Aid • CPR • Other ________________________________________
2. Pass a pre-employment drug screen and random drug screens and maintain emotional control and professional composure at all times.
3. Provide proof or work eligibility status upon request.
4. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years)
5. Demonstrate the ability to:
a. Respond sensitively and competently to the service population's cultural and
socio-economic characteristics.
b. Communicate effectively in writing and verbally in English and Spanish.
c. Work in a fast paced environment.
d. Maintain computer literacy required to meet the responsibilities of the position.
e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
f. Travel as needed.
g. Manage time effectively and efficiently.
6. Bilingual preferred (Spanish)
