LPN Care Manager - Outpatient (Full-time/Cody)
Billings Clinic

Under the general supervision of the Manager, Clinical Coordinator, Director and/or as delegated by the Primary Care RN, assists in providing Care Management services to assigned patients. Works within the scope of licensure as a licensed practical nurse. Applies identified evidence-based nursing practices consistent with national standards and which support quality outcomes. Works collaboratively with the interdisciplinary team to meet the physical, psychosocial, and educational needs of the patient and family. Contributes to the plan of care while maintaining professionalism.

Essential Job Functions

• Supports and models behaviors consistent with Billings Clinic's mission, vision, values, code of business conduct and service expectations. Meets all mandatory organizational and departmental requirements. Maintains competency in all organizational, departmental and outside agency standards as it relates to the environment, employee, patient safety or job performance.
• Conducts chart reviews of each assigned patient per program guidelines.
• Engages with patients and families through phone calls and in person.
• Participates in ED and low-risk Hospital discharge follow-ups as directed.
• Collaborates with other members of the healthcare team through collecting, reporting and recording objective and subjective data; focused review of systems; identifying patient and family priorities; setting realistic and measurable goals; identifying measures to maintain patient comfort; providing health information; reinforcing and participating in the discharge or general plan of care; reinforcing follow-up appointments and assisting as needed in coordinating follow-up appointments.
• Effectively collaborates with Primary Care, Care Management team, Primary Care physicians and office staff in managing shared Primary Care patients and appropriately communicating about those patients with medical or social concerns.
• Provides resource information to patients and support systems as needed per guidelines
• Helps navigate patients and their support system through the continuum of care.
• Participates in health coaching opportunities as available.
• Demonstrates effective communication that is collaborative and respectful toward patients, support systems and colleagues.
• Documentation completed accurately and in a timely manner per Billings Clinic policy.
• Utilizes standards of professional practice in all documentation and communication consistent with organization/department policy as well as the Board of Nursing and ethical guidelines established and universally supported by the nursing profession.
• Documentation and patient information shall be secured and maintained in accordance with Billings Clinic policy, HIPPA, state and federal guidelines.
• Performs all other duties as assigned or as needed to meet the needs of the department/organization.

Minimum Qualifications

Education

• LPN License / Certification

Experience

• Five (5) years of LPN nursing experience (one within the Billings Clinic) preferred

Certifications and Licenses

• Healthcare Provider CPR certifications - At hire
• Current LPN Nurse license in the state of Montana - At hire
