Description: Judge Healthcare is seeking a Personal Care Coordinator in Orange, CA!

Employment type: Contract

Work arrangement: on site

Hours: Monday-Friday, 8a-5p

Role Overview: The Personal Care Coordinator will support Health's Whole Child Model (WCM) procedures. This role involves processing routine member referrals to the Orange County Health Care Agency (OCHCA) for California Children Services (CCS) eligible conditions, as well as handling Annual Medical Reviews (AMR) and Inter-County Transfers (ICT) for WCM members.

Key Responsibilities:

• Collaborate with networks to gather necessary clinical information.
• Coordinate with the OCHCA CCS department to facilitate referral submissions and process AMRs and ICTs.
• Work with members, their families, and the healthcare team to complete Health Needs Assessments (HNA) or Health Risk Assessments (HRA).
• Communicate the member's HNA or HRA and care plan with the member, health network, and healthcare team.
• Identify barriers to care and develop comprehensive care plans at all levels of care.

Minimum Qualifications:

• High School diploma or equivalent.
• 1 year of experience working with pediatric members in a member service capacity.
• Equivalent combinations of education and experience that provide the necessary skills to perform the job duties may also be considered.

Preferred Qualifications:

• Bachelor's degree in healthcare management, social work, human services, or a related field.
• Experience with Health Maintenance Organizations (HMO), Medi-Cal, and Pediatric Health services.
• Bilingual in English and one of the following languages (Arabic, Chinese, Farsi, Korean, Spanish, Vietnamese).
